Position: Bilingual (Spanish/English) Transitions of Care Advanced Practice Provider (APP)

Advanced Practice Provider (NP/PA) – Transitions of Care (TOC) Program

Schedule

Full‑time position, Monday‑Friday, 8:30AM–5:00PM MST/PST. Candidates must be able to work consistently within Mountain Standard Time (MST) or Pacific Standard Time (PST).

Role Overview

The TOC program provides virtual care during the 30‑day window after a patient is discharged from an acute setting (hospital or skilled nursing facility). The clinical team is interdisciplinary, including coordinators, guides, nurse case managers, nurse practitioners, and a medical director. The program’s goal is to prevent readmissions by delivering comprehensive wrap‑around care in the post‑discharge period. The APP will conduct comprehensive post‑discharge assessments, develop individualized care plans, provide disease‑specific education, and manage patients with conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections.

Responsibilities
  • Perform initial TOC visits, including medical assessment, medication reconciliation, order placing (prescriptions, labs, imaging, referrals, home health, DME), disease education, and contingency plans for worsening symptoms.
  • Conduct acute symptom TOC/virtual urgent care visits, diagnosing and managing patients while ensuring appropriate care levels.
  • Participate in interdisciplinary TOC rounds to discuss high‑risk patients.
  • Assist with the clinical education of the overall team.
  • Facilitate safe, open communication across team roles to enhance team culture.
  • Use the electronic health information system for visits and provide interface feedback.
  • Regularly participate in quality improvement projects and pilots.
  • Support new clinical or organizational initiatives as the role evolves.
Qualifications
  • At least 3 years of clinical experience in hospital, post‑hospital, or primary care settings.
  • Clear understanding of common discharge diagnoses, post‑discharge needs, and medical management to avoid readmissions.
  • Ability to work full‑time, 5 days a week, 8‑hour days.
  • Fluency in both Spanish and English.
  • Strong teamwork orientation and desire to work in an interdisciplinary environment.
Desired Skills
  • Experience in internal medicine, urgent care, emergency medicine, hospital medicine, post‑acute care, or geriatrics.
  • Experience performing telehealth visits and managing post‑transition patients.
  • Experience with acute/chronic disease exacerbations (CHF, diabetes, COPD, hypertension).
Licensure & Certification
  • Active RN or APRN license in an authorized state, or active Physician Assistant license.
  • Good standing and willingness to obtain licensure in required states within 90 days of hire.
  • Active BLS certification at hire and throughout employment.
Salary & Benefits

Base salary range: $120,000–$155,000 plus performance‑based bonus ($10,000–$20,000). Total compensation range: $130,000–$170,000.
