We are seeking a dedicated full-time LPN Nurse to join our compassionate team specializing in brain injury assisted living care who possesses management skills. In this role, you will provide oversight to our caregiver team, provide personalized nursing care, administer medications and collaborate with a multidisciplinary team to optimize the quality of life for our residents. Your expertise and empathy will play a crucial role in empowering brain injury survivors through compassionate assisted living care.
Key Responsibilities:
- Deliver high-quality nursing care in accordance with established protocols and individualized care plans.
- Monitor and assess residents' medical conditions, vital signs, and responses to treatment.
- Oversee medications and treatments as prescribed by healthcare providers.
- Collaborate with physicians, therapists, and caregivers to develop and implement comprehensive care plans.
- Educate and routinely update residents and their families on their health status
- Maintain accurate and timely documentation of nursing assessments, interventions, and outcomes.
- Assist residents with self-administration of medication or total medications administration per doctor's orders.
- Assist in procuring, storing, dispensing, inventory and disposing of meds as ordered or appropriate.
- Understands and recognizes how to respond to adverse effects or medication errors. Help keep carts, med rooms organized and clean.
- Available to assist outside visitors, professionals, i.e. hospice, home health agency, coordinates services as needed. Communicate concerns.
- Takes lead in handling all emergency situations performing duties as a first responder and coordinating emergency care as deemed necessary and appropriate. Communicate with paramedics and ER personnel of any transfers to the Emergency Department or hospital.
- Participates in all scheduled team member meetings pertaining to assisted living residents or staff as well as voluntary and mandatory on-going education and in-service training.
- Works as a working manager, assist in guiding departments in meeting resident's needs and ensuring health, safety and welfare as need arises.
- Assist as necessary with resident scheduled and unscheduled daily needs such as ADL's + lADL’s PRN medications.
- Acts as Manager Designee when the Executive Director is not physically present. Able to work weekends.
- Assist in managing resident behaviors, redirect and use PR meds appropriately, encourage activities to prevent episodes or outbursts.
- Know fire census and whereabout of residents always. Watch for elopement risks, participate in drills, know assigned evacuation procedures.
- Practices good infection control principles and uses appropriate hand-washing that is clean, order free and is as risk-free as possible. Notices and reports on changes in residents’ changes or abilities.
- Perform and comply assignments with established procedures and guidelines. Handle emergencies with knowledge and ease. Able to respond in any emergency.
- Guide and support other Care managers. Report care concerns or changes as appropriate to leadership.
- Completes and maintains appropriate documentation of medication and care needs assigned and noted per shift.
- Assist care staff to ensure documentation is completed daily per needs.
- Other duties as assigned by the Executive Director.
Qualifications:
- Licensed Practical Nurse (LPN) with current AZ licensure.
- Experience in brain injury care, rehabilitation or long-term care is preferred.
- Strong clinical assessment and critical thinking skills.
- Strong interpersonal skills and a team-oriented approach.
- Compassionate demeanor and ability to establish rapport with staff, residents and their families.
- Excellent communication and teamwork skills.
- Commitment to upholding ethical standards and promoting resident dignity and autonomy.
- CPR, AED, and First Aid Certification (current and valid)
- Level 1 Fingerprint Clearance Card (must provide proof)
- Proof of being TB-free (2 negative test or current chest X-ray)
- Memory Care Certification (or ability to obtain upon hire)
- Ability to pass the Centralized Background Check
- Strong communication, observation, and documentation skills
- Ability to handle challenging behaviors with empathy and professionalism
