Family Nurse Practitioner (Part‑time)

Position Details

Location:

South Mountain Health Center – 36th St / Baseline, Phoenix, AZ

Hours:

20 hours per week (Outpatient Only)

Qualifications
  • Arizona Family Nurse Practitioner (FNP) license required
  • Certification in Family Nurse Practitioner required
  • FNP experience preferred
  • Experience with Next Gen electronic health records a plus
  • Salary range: MSN – $57,000-$62,000 DOE; DNP – $64,500-$65,000
  • Additional language differential pay available for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili, or ASL at $3 per hour
Responsibilities

The Primary Care Provider conducts physicals and check‑ups, performs medical evaluations and diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, and makes specialty care referrals. The provider educates patients about preventative care and establishes therapeutic relationships to achieve appropriate treatment goals within an integrated setting.

  • Performs independently or as part of a treatment team.
  • Conducts physicals/check‑ups, evaluations, diagnostics, treats illnesses, orders lab tests, and provides general and preventative care, including medication prescribing and monitoring.
  • Educates patients about preventative care, wellness activities, and prescribed treatments.
  • Provides leadership and cohesion within the clinic environment, and may supervise other medical staff and students.
  • Participates in program evaluation and record‑keeping, and makes referrals to specialty and behavioral health care as appropriate.
  • Provides direct patient services at least 85% of the work time.
  • Employs therapeutic interventions appropriate for identified treatment issues.
  • Seeks clinical supervision/support when necessary to ensure effective treatment.
  • Establishes appropriate patient files that conform to agency guidelines, professional practice standards, and state licensing regulations.
