About the Role
Soleo Health is seeking a Full‑time Infusion Nurse to provide infusion therapy care to patients in Phoenix, AZ and the surrounding area.
Position OverviewThe Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting.
Responsibilities- Provide direct care for all assigned patients, including initiation and maintenance of all communications with appropriate care team members.
- Evaluate and educate patient/care partner to safely administer home infusion therapy.
- Evaluate the home environment where the care is provided and identify interventions to ensure safety and appropriateness.
- Develop the plan of care with other team members and evaluate response to therapy.
- Manage schedule via company‑approved platform and communicate any scheduling changes promptly.
- Coordinate patient care via approved company platform.
- Participate in the nurse on‑call rotation to ensure continuous nursing support.
- Document all patient encounters, communications, and care activities at the time of service per Soleo policy and regulatory requirements.
- Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
- Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
- Work with appropriate departments to ensure all required patient documents are completed and returned within an acceptable timeframe.
- Full‑time, 40 hours per week, Tuesday‑Saturday or Sunday‑Thursday, 8:30 a – 5:00 p, flexibility required.
- Must be able to participate in an on‑call rotation.
- Must be knowledgeable of IV access devices (IV lines, ports, medication administration).
- Must be able to drive up to 1 hour to patients’ homes (mileage reimbursement provided).
- Must be licensed as an RN in the state of Arizona.
- Current RN license in good standing.
- RN degree (BSN preferred).
- Advanced certification in specialty areas of practice preferred (e.g., CRNI, ONS, IgRN).
- Experience with on‑call rotation, nights, and weekends.
- Minimum 3 years nursing experience with background in infusion therapy preferred.
- Experience in ICU and/or Cardiac Care preferred.
- Proficiency with Microsoft Outlook and Office Suite.
- Valid state‑issued driver’s license with no significant restrictions.
