Mobile Care LVN; Maricopa, AZ

Overview

SCAN is a nonprofit health organization that serves older adults across the Western United States. This role is for a licensed vocational nurse (LVN) or LPN to provide direct, onsite and remote care to Embrace members residing in assisted living and community‑based settings under the direction of nurse practitioners and physicians.

• Clinical Care & Assessment – conduct routine rounds; check urgent needs; perform focused assessments including vital signs, pain, mental status, basic cardiopulmonary/GI observations, skin integrity, wound status, catheter function and output; identify changes in condition and escalate appropriately.
• Direct Clinical Interventions (within LVN scope) – administer vaccinations and IM/SC injections as ordered, perform wound care (dressing changes, monitoring), routine foot care, urinary catheter care and replacements as per policy, provide peripheral IV insertion, hydration, maintenance and discontinuation; monitor for adverse reactions, complications or lack of response and elevate promptly.
• Transitions of Care & Medication Support – conduct follow‑up check‑ins after transitions or acute events in urgent cases as directed by the clinical team.
• Communication & Care Coordination – communicate frequently and proactively with the primary care team; provide non‑urgent POA/family updates as directed; build strong collaborative relationships with community staff, residents and families.
• Community Engagement & Relationship Development – maintain a consistent, visible presence within assigned communities; partner with community leadership to identify needs and opportunities; support member growth, retention and overall satisfaction; share field insights to inform care delivery and expansion efforts.
• Other duties as assigned; actively support the achievement of SCAN’s vision and goals.

• Licensed Vocational Nurse (LVN) or equivalent qualification.
• Associate’s Degree preferred.
• Three to five years of related experience.
• Ability to work under direct supervision; handle basic issues and problems and refer more complex issues to higher‑level staff; possess a working knowledge of relevant subject matter.
• Proof of tuberculosis screening (negative test) within the last year, or proof of screening upon hire, is required.

• Base salary range: $30.77 to $44.52 per hour.
