• Provide skilled nursing care under the direction of an RN.
• Administer medications and treatments based on individualized plans.
• Educate and support families in-home care techniques.
• Observe and report changes in patient condition.
• Document visits using EMR system.
• Follow the nursing care plan approved by RN/physician.
• Assist in specialized procedures and self-care education.
• Participate in patient care conferences and quality improvement activities.
• Take on-call duty, nights, weekends, and holidays as assigned.

• Active LPN license.
• CPR certification.
• Excellent communication and critical‑thinking skills.
• Passion for working with pediatric and/or adult patients in a home setting.
