Position Summary
We are seeking an advanced practice provider (nurse practitioner or physician assistant) to support the inpatient nephrology service, working closely with physicians, nurses and interdisciplinary teams to provide high‑quality care for hospitalized patients with acute and chronic kidney disease. The APP will play an integral role in care coordination, patient management, family education and clinical collaboration across intensive care units and inpatient medical services.
Key Responsibilities- Provide comprehensive inpatient nephrology care in collaboration with pediatric nephrologists.
- Evaluate, diagnose and manage kidney‑related disorders in hospitalized children.
- Support pediatric patients requiring dialysis and CKRT.
- Coordinate care plans with multidisciplinary teams across inpatient units.
- Participate in daily rounds, discharge planning and patient/family education.
- Assist with quality improvement and evidence‑based practice initiatives.
- Contribute to a collaborative teaching environment.
- Conduct comprehensive health history, physical assessments and plans for infants, children and young adults with acute and chronic health conditions.
- Order and prescribe pharmacologic and non‑pharmacologic interventions, and perform other therapeutic or corrective measures to treat illness and improve health status.
- Formulate family‑centered plan of care and perform daily documentation as necessary.
- Collaborate and coordinate care with other team members.
- Collect and analyze data when necessary.
- Perform any needed advanced procedures aligned with credentialing process (DOP).
- Utilize up‑to‑date evidence‑based practice for plan of care and adapt it according to family needs and effectiveness.
- Serve as an educational resource to patients/families/nursing staff/provider team and other healthcare teams.
- Participate in professional development by maintaining required competencies, certifying body renewals, seeking educational offerings and guiding the professional development of others, including advanced practice provider students, residents and medical students. Act as preceptor/mentor for new staff.
- Demonstrate commitment to patient safety by raising concerns in an appropriate manner, reporting through appropriate channels and ensuring change of practice when necessary. Demonstrate shared accountability for safe practice.
