Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona. CBI serves individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

As a Psychiatric Mental Health Nurse Practitioner (PMHNP) in crisis, you will provide patient‑centered, evidence‑based care and serve as a patient advocate across all health‑related matters, including appropriate use of specialists, health services, and community resources.

  • Inpatient settings: Evaluate adults 18 and older, all genders, all socio‑economic backgrounds, complex medical history, and co‑occurring psychiatric disorders.
  • Outpatient settings: Provide assessment, diagnosis, treatment recommendations, and coordinate care transitions.

Shift options:

  • PMHNP/FNP – Crisis Sobriety Center – AM/PM shifts (Sunday – Tuesday rotating, Wed – Thursday – Saturday).
  • PMHNP – SMI Caseload – Full time on site, Mesa location; 8 AM‑4 PM Monday – Friday.
  • PMHNP – ACT/FACT – Full time on site, Central Phoenix; 8 AM‑5 PM Monday – Friday.

Core Responsibilities:

  • Complete urgent psychiatric and/or addiction medicine evaluations for patients referred to the 23‑hour observation unit from hospitals, urgent care, law enforcement, and other practitioners for assessment, diagnosis and treatment recommendations.
  • Conduct urgent psychiatric assessments, addiction medicine, ambulatory detox, and physical health assessments as identified by CBI screening staff.
  • Coordinate admissions, transfers, and discharges within the CBI program and document transitions for smooth care continuity.
  • Initiate and maintain documentation of doctor‑to‑doctor communication and coordination of care.
  • Evaluate all patients at least daily on the observation unit to adjust treatment and discharge planning.
  • Document and sign full discharge orders, follow‑up recommendations, and discharge plans within 24 hours of discharge.
  • Write and sign admission orders within 24 hours of admission and coordinate with RN and crisis specialist during rounds.
  • Maintain treatment of physical health and coordinate with PCP and other prescribers.
  • Carry out seclusion and restraint interventions per CBI policy, including assessment, order documentation, and post‑intervention documentation.
  • Support universal requirements such as orientation, TB skin testing, Suboxone waiver maintenance, CPR/First Aid certification, documentation compliance, and quality management initiatives.
  • Participate in staffing rounds, interdisciplinary team meetings, utilization review, and regulatory or accreditation surveys.
  • Maintain minimum performance metrics: 85% documentation compliance, 85% patient satisfaction, 75% follow‑up rate.
  • Ensure compliance with session scheduling protocols, including advance notice for time‑off and emergency coverage communication.
  • Other duties as assigned per CBI policy.

Qualifications:

  • Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) or Family Nurse Practitioner (FNP) with current licensure.
  • Experience in crisis, inpatient, or outpatient behavioral health settings.
  • Competence in conducting psychiatric evaluations, substance use assessments, and coordinating multidisciplinary care.
  • Familiarity with seclusion and restraint protocols and documentation.
  • Strong communication skills, ability to advocate for patients, and maintain interdisciplinary collaboration.
  • Compliance with CBI’s minimum documentation, quality, and patient satisfaction standards.
