Nurse Practitioner

Welcome Home House Calls is seeking Nurse Practitioners and Physician Assistants to provide in-home care to patients recently discharged from hospital settings. The role involves ensuring a smooth transition back home and facilitating patients’ return to daily activities while establishing one‑to‑one relationships in a patient‑centered environment.

• Flexibility:
  Make your own schedule! We coordinate availability 2 weeks in advance.
• Limited Geographic Areas – because traveling from Peoria to Apache Junction is not ideal.
• No call schedule.
• Direct Physician Support – colleagues available for clinical questions and to manage prescriptions, diagnostics, and other administrative tasks so you can focus on patient care.
• RN Case Management – available for patient coordination, questions, and to ensure patients receive everything they need when you are not on site.
