American Premier NP2U | Full-Time | Field-Based

American Premier is seeking a compassionate and clinically strong Nurse Practitioner to join our growing palliative care program, NP2U. This role serves patients with serious illness in community-based settings, including assisted living communities, memory care, group homes, and private residences.

This position is ideal for a provider who values meaningful, relationship-based care and is comfortable managing medically complex patients outside of a traditional clinic setting. The NP2U Palliative Care Nurse Practitioner provides comprehensive assessments, symptom management, diagnostic evaluation, treatment planning, advance care planning, and care coordination for patients and families navigating serious illness.

This is a highly autonomous role requiring strong clinical judgment, excellent communication skills, and comfort working independently in the field.

Position Overview

The Palliative Care Nurse Practitioner is responsible for evaluating and managing patients enrolled in the NP2U palliative care program. The provider will conduct in-person visits, assess changes in condition, manage symptoms, order and interpret diagnostic testing, prescribe medications, develop care plans, and coordinate recommendations with the patient's broader care team.

The provider may also complete Medicare-required hospice Face-to-Face encounters for hospice patients, supporting recertification and documentation of continued hospice eligibility.

Key Responsibilities

Clinical Care

  • Provide in-person palliative care visits for patients in assisted living, memory care, group homes, private residences, and other community-based settings.
  • Perform comprehensive assessments and follow-up evaluations for patients with serious, chronic, or progressive illness.
  • Assess changes in condition and determine appropriate clinical interventions.
  • Manage complex symptoms including pain, dyspnea, nausea, anxiety, weakness, functional decline, edema, wounds, behavioral symptoms, constipation, anorexia, insomnia, and other distressing symptoms.
  • Develop and update individualized treatment plans based on patient condition, prognosis, goals of care, and clinical findings.
  • Order and interpret labs, imaging, and other diagnostic studies as appropriate.
  • Prescribe and adjust medications within scope of practice.
  • Provide education to patients and families regarding disease progression, symptom management, medications, treatment options, safety concerns, and available support services.
  • Facilitate goals‑of‑care discussions and advance care planning conversations with patients and families in a compassionate and patient‑centered manner.
  • Coordinate with physicians, specialists, hospice, home health, facility staff, pharmacies, families, and other members of the care team as clinically appropriate.
  • Document all clinical encounters accurately and timely in the electronic health record in compliance with regulatory and payer requirements.

Hospice Face-to-Face Encounters

The NP may serve as a qualified provider for Medicare-required hospice Face-to-Face encounters.

Responsibilities include:

  • Conduct in-person Face-to-Face visits for hospice patients prior to the third benefit period recertification and each subsequent recertification, as required by Medicare.
  • Perform a thorough clinical assessment to evaluate and document current status, decline, disease progression, and continued hospice appropriateness.
  • Complete required Face-to-Face documentation within established time frames.
  • Communicate clinical findings to the hospice interdisciplinary team and certifying provider as needed.
  • Ensure documentation supports the clinical narrative for continued terminal prognosis and hospice eligibility.
  • Maintain compliance with Medicare and payer‑specific requirements related to hospice Face‑to‑Face encounters.

Compliance & Documentation

  • Maintain accurate and timely documentation supporting assessment, medical decision‑making, treatment plans, symptom management, and care coordination.
