Position Overview:
  • We are seeking a Full‑Time Nurse Practitioner (NP) to join our dynamic and supportive team.

Key Responsibilities:
  • Perform patient assessments, physical exams, and diagnostic evaluations.
  • Diagnose and treat acute and chronic health conditions.
  • Prescribe medications and manage treatment plans.
  • Educate patients on health promotion and disease prevention.
  • Collaborate with physicians, behavioral health providers, and care coordinators.
  • Participate in chronic care management and MAT services as appropriate.
  • Maintain accurate and timely documentation in the electronic health record (EHR).
  • Engage in ongoing quality improvement and team‑based care initiatives.
Qualifications:
  • Current and unrestricted Nurse Practitioner license in Arizona, or eligible for licensure in AZ.
  • Board certification as a Family or Adult NP (FNP, ANP, or AGNP).
  • DEA license and prescriptive authority.
  • Minimum of 1–2 years of primary care experience preferred (new grads considered).
  • Experience with MAT, behavioral health, or chronic care is a plus.
  • Strong communication and clinical skills.
  • Proficiency with EHR systems (eClinicals preferred).
  • Bilingual (Spanish, French, or other languages) a plus.
