Position Summary

As a LPN in this clinic, you will provide patient care under the direction and supervision of a licensed physician, and be accountable for the quality of services delivered by yourself or others under the physician’s direction. This position handles medication refills, prior-auths, calls patients with lab results, and manages a heavy workload that includes daily provider and patient messages.

This is a full-time (40 hours/week) day shift position:
Monday
- Friday 8:30 A - 5:00 P.

Core Functions
  • Observe, compile and report the patient’s physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Review available information obtained by health care team members.
  • Contribute to the plan of care under the direction of a registered nurse, including the discharge plan, using assessment data and input from the patient, family and health team. Plan care in collaboration with multidisciplinary team members and provide updates as needs change.
  • Implement care based on delegated interventions in the care plan and medical orders, within scope of practice. Provide care per physician orders and the nursing plan of care, complying with policies, procedures, standards of care and regulatory requirements. May delegate and supervise non-licensed personnel as allowed by state scope of practice. Promote continuity of care by communicating patient status to other caregivers.
  • Report patient condition, including signs and symptoms that may indicate changes, to the registered nurse and/or physician. Observe and report progress toward goals and expected outcomes, and patient response to teaching.
  • Document observations, interventions and data in the patient record. Document reflects objective/subjective data, nursing interventions and patient responses. Record physician orders accurately and timely.
  • Plan, organize, develop and coordinate documentation and input of patient assessment data as required by federal regulation. Maintain, update and revise documentation per patient changes, following federal and state guidelines. Communicate and coordinate with interdisciplinary team about functional/care level and transitions. Perform audits related to reimbursement documentation and quality management.
  • Coordinate the admission process, including new resident evaluations. Provide verbal and written communication to residents, families and referral sources about admission procedures. Facilitate problem solving with residents, families, referral sources, insurance companies and clinical staff.
  • Work under supervision, prioritizing data from multiple sources to provide quality care and support to patients and families. Operate in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interact with staff at all levels, patients, families and physicians.
  • Minimum Qualifications

    Must possess a current, valid LPN or LVN license in the state of practice, a temporary LPN or LVN license in the state of practice, or compact LPN or LVN licensure for the current state of practice.

    BLS certification is required.

    Additional certification or continuing education may be required based on area of practice.

    Completion of a state-approved IV therapy program may be required in certain facilities and/or departments as per state scope of practice, regulatory requirements or facility policy.

    Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a LPN or LVN in the specialty area. Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment.

    Preferred Qualifications

    Experience in a designated clinical area is preferred.

    Additional related education and/or experience preferred.
