Position Title

RN Orthopedic Navigator

Position Summary

As an RN Orthopedic Navigator, you will support the elective and non‑elective orthopedic service line and provide coordination services for Thunderbird’s growing ortho‑trauma services. The role provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. You will collaborate with a multi‑disciplinary team to facilitate timely screening, diagnosis, treatment, and supportive care throughout the patient experience.

Core

Functions

Assess the patient’s plan of care, develop, implement, monitor, and document the utilization of resources, and facilitate options and services to meet the patient’s healthcare needs.

Evaluate the medical necessity and appropriateness of care, optimize patient outcomes, serve as a liaison for referring physicians, and assist with scheduling initial tests and consultations.

Assist the patient/family through diagnostic services, treatment, and care; coordinate disease care using an interdisciplinary holistic approach, and make appropriate referrals and consultations.

Ensure the patient and family understand diagnostic processes, care strategies, and recommended actions, and provide appropriate information in coordination with physicians and healthcare providers.

Evaluate patient’s functional abilities and limitations, establish treatment goals, develop and implement a plan of care, collaborate with family on goal setting, initiate discharge planning, and recommend modifications to referring orders.

Support patients and families during difficult decision‑making periods, assist in coordinating end‑of‑life care, and provide emotional support.

Support process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomes; participate in staff development to maintain current standards.

Establish and promote collaborative relationships with physicians, payers, and other members of the healthcare team; collect and communicate pertinent information to fulfill utilization and regulatory requirements.

Work independently, determine how to best accomplish functions within established procedures, and confer with supervisor on unusual situations. The position is facility‑based with no budgetary responsibility.

Minimum Qualifications

Bachelor’s degree in nursing or related field.
Current RN license in the state of practice.
BLS certification required.
Five years of clinical experience.
Excellent organizational skills and clinical knowledge regarding specialty care services.
Experience in care coordination of diagnostic and health services, including legal and financial aspects.
Effective communication and writing skills.
Good time‑management and proficiency with word‑processing and database software.
Ability to teach both clinical and non‑clinical personnel regarding care and diagnostics services.
Understanding of process improvement principles.

Preferred Qualifications

Current certification in specialty area preferred.
Prior case management experience preferred.
Additional related education and/or experience preferred.
