We are seeking a Licensed Practical Nurse (LPN) to join our evening shift team at our Phoenix, Arizona location. As an LPN, you will be responsible for providing direct patient care, administering medications, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

• Provide direct patient care, including vital sign checks, wound care, and medication administration
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor patients' conditions and report any changes or concerns to the nurse manager or physician
• Assist with daily living activities, such as bathing, dressing, and toileting
• Collaborate with other healthcare professionals to ensure seamless transitions of care
• Maintain accurate and detailed records of patient care and medications
