Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience

Roles & Responsibilities:

• Assess patients in various situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and families regarding care, diet, medications, and home safety using appropriate teaching methods.
• Supervise and provide clinical direction to other healthcare staff to ensure quality service delivery.
• Monitor assigned cases for compliance with third-party payer requirements.
• Provide effective communication as a liaison between patients/families, team members, and other healthcare professionals.
