Registered Nurse - Transplant PCU
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The RN is accountable for the quality of nursing services delivered by self or others under their direction and utilizes specialized knowledge and judgment to assess data and plan, provide and evaluate care appropriate to the patient’s physical and developmental age.
CoreFunctions
Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and ensures completion within the appropriate timeframe, including reviewing diagnostics and laboratory data and reporting abnormal results to the physician and other caregivers. Completes assessments and reassessments according to patient need and policy.
Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan based on patient-specific needs and the medical plan of care. Goals for the patient are realistic, measurable and developed in conjunction with the patient/family, considering physical, cultural, psychosocial, spiritual, age‑specific and educational needs. Plans care in collaboration with multidisciplinary team members and revises the plan as patient status changes.
Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Provides care based on physician orders and the nursing plan, in compliance with policies, procedures, standards of care and regulatory requirements. Delegates appropriately and provides nursing supervision to other licensed nurses and personnel. Promotes continuity of care by accurately and completely communicating patient status to other caregivers.
Evaluates the patient’s and family’s response to care and teaching, revises the plan as needed, and evaluates patient progress toward goals and expected outcomes in collaboration with other health care team members.
Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy and reflects objective/subjective data, nursing interventions and patient responses. Records physician orders accurately and in a timely manner.
Provides care based on the best evidence available and participates in research activities within clinical practice. Participates in unit or facility shared leadership and in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Supports growth of others through precepting and mentoring when appropriate.
Contributes to society through activities that lead to excellent patient outcomes, including participation in national nursing quality indicators and professional organizations.
May supervise staff and workflow of the department in some roles.
Interacts with all levels of staff, physicians, patients, families and external contacts regarding patient care, including clergy for spiritual support. Synthesizes and prioritizes data from multiple sources to support the patient and family response to changes in health status.
- Current, valid RN license in the state of practice (or temporary/interim license, or compact RN licensure).
- BLS certification required.
- Minimum one‑year experience in an acute care hospital setting, including recent experience as an RN in the specialty area.
- Experience in the clinical area for which the RN is applying is desired.
- Bachelor’s degree preferred.
- Professional certification preferred.
- Additional related education and/or experience preferred.
