Registered Nurse; Home Health

What is the role?

On behalf of our home health agency client, one of the largest agencies in the Phoenix Area, we are looking for multiple Registered Nurses to care for and focus on patients in a small radius around the candidate's residence. As a home health nurse, you’ll work one-on-one with your patients in the comfort of their homes, developing deeper relationships in a personal setting.

• Self-manage, schedule and perform visits based on assigned patient base
• Complete an initial home health assessment of patient and family to determine home care needs
• Provide a complete physical assessment and history of current and previous illness
• Conduct home visits as assigned, assess the patient's condition, progress toward goals, and provide patient care, teaching and other skilled interventions as ordered
• Regularly re-evaluate patient nursing needs as part of nursing visits
• Initiate appropriate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by the physician
• Provide health care instructions to the patient as appropriate per assessment and plan of care
• Communicate needs to the team as part of the care plan development; assist in implementing before discharge of the patient
• Inform personnel of changes in the condition and needs of the patient
• Counsel the patient and family/significant others in meeting nursing and related needs
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients promptly as per Agency policy
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition by Agency defined time frames
• Assure proper maintenance of clinical records in compliance with local, state and federal laws

• Graduate of an accredited school with at least one (1) year of nursing experience
• Preference will be given to candidates with experience working in home health, hospice & skilled nursing, but not required
• Ability to autonomously work with little supervision
• Passionate about patient care and making an impact by treating patients in their own homes
• Must have an Arizona license in good standing and a Driver's License
• Reliable transportation, current CPR or BLS Card

We are looking for candidates across the entire Phoenix Area. Candidates will be grouped in 3 regions, from West to East, and will travel to see patients within a 25-mile radius of their place of residence.
