Location: 337 East Coronado Road Suite 101, Phoenix, Arizona.

Local DaVita SNF dialysis dens are located at Haven Sky Harbor, Palm Valley, and Resolve. The RN works in a 6‑chair dialysis den. It is likely a 3‑day per week schedule with NO on‑call. This role is open for census growth and the RN must be flexible to support any of our local SNF Dialysis dens and possibly receive some hours in outpatient dialysis during census growth periods.

In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.

RNs by training are educators. As a DSD Services RN you will conduct impactful and meaningful education to patients and their families in the SNF setting.

Direct Patient Care:
The RN’s primary responsibility is to independently demonstrate best practice techniques and deliver optimal dialysis care. The role includes supervising Dialysis Technicians who are tasked with delivering tandem care to meet capacity and efficiency goals.

Complexity: DSD Dialysis care is complex but fulfilling. Meeting and/or exceeding clinical outcomes are the goals, and multitasking is an expectation to meet the needs of the service, patients, and our SNF partners.

Autonomy and Critical Thinking:
Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times, apply core nursing skills to assess, diagnose, and implement complete care plans, and manage multiple priorities.

• Current Registered Nurse (RN) license in the state of practice; minimum of 12 months RN experience.
• Associate Degree in Nursing (ADN) from an accredited school of nursing required;
  Bachelor of Science in Nursing (BSN); a 3‑year diploma from an accredited diploma program may be substituted for a nursing degree.
• Basic computer skills and proficiency in MS Word and Outlook.

• Previous Chronic Hemodialysis, Acute Dialysis, and/or Peritoneal Dialysis (PD) nursing experience.
• Training/Teaching/Healthcare Education background.
• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
