Banner Health is seeking a Registered Nurse (RN) Heart Transplant Coordinator to join the Heart Transplant team at Banner University Medical Center (BUMC) Phoenix. This is a day shift position working Monday through Friday, 7:00 AM to 4:30 PM with some schedule flexibility, plus 24-hour on-call rotations (including weekend and holiday coverage). In this position, the RN Heart Transplant Coordinator will work with both pre and post-heart transplant patients while collaborating closely with the multidisciplinary team. The role requires an active and engaged change agent who is dedicated to the needs of patients and families, committed to delivering safe, kind, effective, efficient, evidence-based, and high-quality clinical care. The coordinator will focus on optimal outcomes and solution-oriented approaches while building strong relationships with patients throughout their transplant journey.

Banner University Medical Center Transplant Institute is Arizona's most experienced transplant center, providing comprehensive care to adult patients across heart, lung, liver, and kidney/pancreas transplant programs.

The ideal candidate will have cardiac nursing experience (critical care or progressive care). Preferred, but not required, additional experience of value includes: transplantation, care coordination and mechanical circulatory support devices. POSITION SUMMARY

This position provides comprehensive management and coordination of care for patients requiring kidney, pancreas, liver, heart, lung or Stem Cell Transplantation. The level and intensity of care coordination provided is situational and appropriate based on patient need and payor requirements.

CORE FUNCTIONS
  • Manages individual patients across the transplant health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes. Facilitates, coordinates, and monitors pre‑transplant evaluations, the care of the transplant candidate awaiting transplantation, the care of the transplant recipient post transplant in the outpatient clinic. Facilitates and coordinates the care of the living donor for evaluation, donation and long‑term follow‑up. Collaborates effectively with the interdisciplinary team to plan and implement the patient’s treatment plan, utilizing effective oral and written communication.
