As an RN Case Manager, you are the heartbeat of our multidisciplinary care team. Your day-to-day includes:
- Developing and executing individualized care plans tailored to each patient’s medical, functional, and emotional needs
- Leading interdisciplinary collaboration with PTs, OTs, social workers, home health aides, and physicians
- Conducting in-home patient assessments and adjusting care plans as conditions evolve
- Educating patients and families on disease management, medications, and self-care
- Completing accurate, timely EHR documentation in compliance with Medicare and state regulations
- Tracking KPIs including visit completion rates, clinical outcomes, and patient satisfaction scores
- Reducing hospital readmissions and improving functional independence through evidence-based care
- Education: Associate’s or Bachelor’s degree in Nursing (BSN preferred)
- Licensure: Active RN license in Arizona (required)
- Experience: 2+ years of clinical nursing experience; 1+ year in home health preferred
- Skills: Strong assessment, care planning, time management, and communication skills
- Tech: Comfortable with EHR systems; home health software experience is a plus
- Regulatory Knowledge: Familiarity with Medicare/Medicaid home health standards
- Mindset: Patient advocate, team player, and adaptable in a fast‑paced field environment
- Primarily in patients’ homes throughout the Phoenix metro area
- Occasional office visits for team meetings and training
