This is a full‑time position that will service patients across Scottsdale and Fountain Hills. Annual salary range is $85,000 – $95,000 per year (depending on experience), plus travel expenses.

• Build meaningful relationships with patients and families.
• Create personalized care plans that truly make an impact.
• Coordinate care as a trusted case manager.
• Educate and empower patients and caregivers with confidence.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.

• Current RN license in the State of Arizona.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient‑centered care.
