Position Summary

Position: Pre‑Admission Testing Nurse (PAT) – Preadmission Services, Phoenix, Arizona.

Employment Type: Full‑time.
Hours per Week: 4–10.
Shift Time: 0800‑1830, Monday‑Friday.
Schedule: Varied.
Call: None.
Weekend/Differential Pay: Flat rate of $5/hr for call (if applicable), $3/hr weekend shift differential, and 18% night shift differential when applicable.

Core Functions

Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs; review patient history and physical, and ensure timely completion.

Formulate a patient‑specific plan of care, including discharge plan, based on assessment data and collaboration with patient, family, and health team.

Implement the plan of care through direct patient care, coordination, delegation, and supervision of health‑care team activities while complying with policies, procedures, and regulatory requirements.

Evaluate patient and family response to care and teaching, revise plan of care as necessary, and assess progress toward goals and outcomes.

Document assessment, planning, implementation, and evaluation in the patient record, ensuring legibility, timeliness, and accuracy of objective and subjective data, nursing interventions, and patient responses.

Provide evidence‑based care and participate in research, unit leadership, and education/mentoring activities as appropriate.

Contribute to improvements in nursing quality indicators and participate in professional nursing organizations.

In some roles, supervise staff and work flow within the department.

Interact with all levels of staff, patients, families, external contacts, and clergy to provide comprehensive care and support.

Minimum Qualifications

Current, valid RN license in the state of practice, temporary/interim RN license, or compact RN license for the state of practice.
BLS certification required.
Experience in the clinical area for which applying is desired.
Annual validation of clinical performance competencies for the specialty area.
Banner Registry and Travel acute‑care positions require a minimum of one year experience in an acute‑care hospital setting, including work within the past 12 months as a Registered Nurse.
Employees in home‑health settings must operate a personal vehicle, possess a valid driver’s license, and maintain active auto insurance coverage.
Firearm clearance requirements apply to specific behavioral health and senior behavioral health positions as per state regulations.
Arizona Criminal History Affidavit completed upon hire for positions in Arizona.

Preferred Qualifications

Bachelor’s degree in Nursing or related field.
Active professional certifications relevant to the specialty.
Additional related education and/or experience preferred.
