In-Home Nurse Practitioner or Physician Assistant Part Time - Cincinnati

Overview

Signify Health, part of CVS Health, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The role involves engaging with patients in the comfort of their homes, conducting clinical assessments, and delivering comprehensive, personalized health evaluations that connect patients with the right care.

• Conduct 45‑minute home visits that include medication and medical history review, a physical evaluation, and, when ordered by the patient’s health plan, diagnostic tests such as diabetic eye exams and spirometry.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Maintain professionalism and safety in diverse home environments, including households with pets, second‑hand smoke, or allergens.
• Travel locally and within licensed states as required.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated in the scope of practice, including Diagnostic & Preventive Services such as spirometry, DEE, and bone density screening.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Anticipated weekly hours: 29.5

Pay range: $37.66 – $81.11 per hour (or equivalent full‑time salary)
