The PositionUsing strong clinical and assessment skills to assess patients’ post‑acute needs in collaboration with the multi‑disciplinary Halos Home Team and the patients’ discharge care providers to ensure continuity of care when discharging home.
Essential Duties and Responsibilities (Other Duties May Be Assigned)- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel. Maintain positive internal and external customer service relationships.
- Conduct ongoing patient care and assessments.
- Administration of prescribed medications, treatments, and therapies.
- Coordination of care.
- Educate family members/caregivers on patient clinical care to enhance positive outcomes.
- Preventative initiatives to protect quality of care for patient.
- Assess patients’ post‑acute care needs in collaboration with the Clinical Care Managers and the patients’ discharge care providers to ensure continuity of care when discharging home.
- Provide interim care to patients experiencing gaps due to call‑offs, vacations and staff shortages with varying acuity and scheduling needs.
- Provide one‑on‑one training to nurses in the field to ensure a smooth clinical and non‑clinical transition assuring continuity of care when the Halo’s Home Team exits the case.
- In conjunction with support staff, serve as a liaison and patient advocate on complicated cases.
- Exercise necessary cost control measures.
- Maintain positive internal and external customer service relationships.
- Demonstrate effective communication skills by conveying necessary information accurately, listening effectively and asking questions when clarification is needed.
- Be depended upon to plan and organize work effectively and ensure its completion.
- Meet all productivity requirements.

- Active PA RN or LPN license.
- Active CPR certification.
- Trach/vent experience preferred.
- Home care preferred.
- Strong oral and written communication skills.
- Adaptability.
- Provide care in a client home setting.

- Ability to work as a team member.
- Flexibility in scheduling with the ability to work on‑call as needed.
- Ability to lift up to 50 pounds and move. Work may involve stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition to meet patient/family individualized psycho‑social needs.
- Office: routine office environment with moderately high noise level, flexible schedule and extended hours. Must be able to travel locally and some exposure to inclement weather. Must respond to pages and understand and apply training and in‑service education, instruct personnel during training, education and staff meetings, remain calm in emergency situations and handle multiple tasks. Must cope with the mental and emotional stress of the position.
