Position Overview
Banner Health is looking for a Pediatric RN Case Manager to provide telephonic complex case management to children and families within the Maternal & Child Health team. The role includes partnering with members, caregivers, providers, and community resources to coordinate care, address barriers, and support the management of complex medical needs.
Work Schedule & LocationTraining is onsite for a minimum of three months at Banner Corporate Mesa or Banner Corporate Tucson. After training, the position is remote with a Monday – Friday, 8am – 5pm schedule. Candidates must live in Arizona to be considered.
Position SummaryThis role provides comprehensive care coordination for assigned patients, assessing care plans, and implementing utilization of resources. The RN Case Manager monitors progress and facilitates options to meet patient healthcare needs while ensuring quality clinical services.
Core Functions- Manages individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Leads process improvement activities for patient populations to optimize outcomes.
- Collaboratively develops and manages interdisciplinary patient discharge plans and communicates the plan across the continuum of care.
- Evaluates medical necessity and appropriateness of care, optimizes patient outcomes, and identifies issues that may delay discharge.
- Establishes and promotes collaborative relationships with physicians, payers, and other members of the healthcare team, collecting and communicating pertinent information to meet utilization and regulatory requirements.
- Educates internal team members on case management and managed care concepts, facilitating integration into daily practice.
- May supervise other staff as needed.
- Determines how best to accomplish functions within established procedures, consulting the supervisor on unusual situations.
- Bachelor’s degree in case management or health care, or equivalent experience.
- Current Registered Nurse (RN) license in Arizona.
- 3–5 years of clinical experience, with proficiency in acute care, home care, or community resources.
- Knowledge of care management, resource utilization, and acute care settings.
- Strong critical‑thinking, problem‑solving, effective communication, and time‑management skills.
- Ability to work effectively in an interdisciplinary team format.
- For acute‑care assignments, willingness to work flexible hours and take rotating after‑hours call.
- For Banner registry and travel positions, at least one year of acute‑care or home‑care case‑management experience, including work in the past 12 months.
- Certified Case Manager (CCM) certification.
- Additional related education or experience preferred.
$35.43 – $59.05 per hour, based on experience, education, certifications, and geographic location.
