Provide direct, compassionate patient care within our Assisted Living facility;
Assist with activities of daily living (ADLs) including bathing, oral care, feeding, ambulation, and transfers;
Monitor and accurately record patient vital signs, weight, positioning, ADLs, I&O, and blood glucose readings;
Report abnormalities to the nursing staff or physician;
Communicate patient needs and changes with RN, physician, social worker/case management, and chaplain services as appropriate
