Position Summary
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The role is accountable for the quality of nursing services and utilizes specialized knowledge, judgment, and nursing skills necessary to assess data, plan, provide, and evaluate care appropriate to the physical and developmental age of assigned patients.
Core Functions
Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information from other health care team members and diagnostics/lab data. Reports abnormal results to physician(s) and other appropriate caregivers. Completes assessment and reassessments as needed.Formulates a plan of care, including discharge plan, using assessment data and input from patient, family and team. Initiates care plan based on patient‑specific needs. Sets realistic, measurable goals in conjunction with patient/family. Considers physical, cultural, psychosocial, spiritual, age‑specific and educational needs. Reviews and revises plan as patient needs change.Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Provides care per physician orders and nursing plan, adhering to policies, procedures and regulatory requirements. Delegates appropriately and provides nursing supervision to other licensed nurses and personnel. Promotes continuity of care by communicating accurately and completely with other caregivers.Evaluates patient and family response to care and teaching, revises plan of care as needed. Evaluates progress toward goals and expected outcomes with other team members. Evaluates patient response and effectiveness of patient teaching.Documents assessment, planning, implementation and evaluation in patient record. Documentation is legible, timely and aligns with policy. Reflects objective/subjective data, nursing interventions and patient response. Notes physician orders accurately and timely.Provides evidence‑based care and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts in education, role development and orientation of facility personnel, patients, students, families and visitors. Supports growth of others through precepting and mentoring when appropriate.Contributes to society through activities that lead to excellent patient outcomes by providing timely, effective, efficient, equitable and safe care. Actively participates in improvement of national nursing quality indicators and outcomes. May participate in professional organizations.May supervise staff and workflow of the department.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions and community providers, concerning the health care of the patient. Interacts with other health care providers in numerous settings to report and clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to support patient and family response to changes in health status.Minimum
Qualifications
- Valid RN license in state of practice (or temporary/interim RN license or compact RN licensure) in the current state of practice.
- BLS certification required.
- Additional certification or continuing education may be required based on area of practice.
