Registered Nurse (RN) Case Manager
We are seeking support from contract nurses to assist with the increasing nursing caseloads. Locations will vary, and the duty post will be defined after candidate selection. The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered. Assessments are used to authorize skilled nursing services for members. The successful candidate will conduct in-person visits statewide and monitor health needs.
They will assist with referrals and advocate for member rights. The Registered Nurse will complete a report for each member, documenting medical necessity for skilled services. In-person assessments will be performed every 90 days and as needed. This position is field-based, requiring travel to complete assessments. The nurse will complete documentation in the timeframe established by relevant guidelines. They will collaborate internally and externally in multidisciplinary planning meetings.
The role also includes completing nursing referrals and placing vendor calls. Special projects may be assigned as needed. Mandatory meetings and required annual trainings are expected to be attended.
