JOB SUMMARYOzanam Manor (I and II) Interim Housing Communities are transitional housing programs for 160 single adults experiencing homelessness who are 50+ or disabled, including Veterans. Nursing care is necessary for homeless Veterans who have some type of unstable or uncontrolled medical issue that requires a level of oversight. The Ozanam Nurse’s primary responsibility is to provide designated Veterans with high quality medical and mental health recuperative care in accordance with the Veteran’s Administration Medical Recuperative Care Contract (approximately 20 hours per week).
For the remainder of the position, the Ozanam Nurse is responsible for the overall healthcare oversight for the program, primarily through consultation to staff regarding Resident medical concerns, but with more direct oversight for residents with high medical needs.
- Enjoys working as part of a team of staff and volunteers and can think and act quickly when there is a need to pivot or address an emergency.
- Has a strong knowledge of chronic illnesses, geriatrics and medications.
- Can work independently and manage own work schedule while ensuring complete and thorough communication with team members and fulfillment of job duties.
- Is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow through from residents.
- Is energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations.
- Exhibits the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment.
Medical Recuperative Care Contract Resident Veterans
(No less than 20 hours per week for up to 5 Residents)
The Ozanam Nurse is responsible for following the plan of care developed by VA referring healthcare providers. This includes medication management, monitoring vital signs and coordination of care with VA medical staff. The services to be provided shall include but are not limited to:
- Meeting with all newly admitted assigned veterans within 72 hours to complete a nursing assessment that includes self‑care skills and to document prescribed medication and orders.
- Providing weekly education and assistance with medication management and treatment compliance within the scope of practice for nursing.
- Monitoring vital signs and following up with VA medical staff as appropriate within the scope of practice for nursing.
- Providing assistance for minor procedures (e.g., dressing change, irrigation) within the scope of practice for nursing.
- Providing health education in areas such as sleep, nutrition, hygiene, coping with chronic illnesses and pain management.
- Assisting Veterans with the use of VA telehealth as needed.
- Overseeing and assisting with coordination of Veteran participation in in‑home health care and outpatient services to address varying recuperative needs (i.e., Physical Therapy, Occupational Therapy, Wound Care, etc.).
- Supporting coordination of care with VA providers and the VA Liaison to ensure continuity of care.
- Maintaining a timely and thorough electronic clinical record that documents medical prescriptions and orders issued by physicians and that documents nursing notes in a professional manner that utilizes clear goal setting and progress towards those goals (e.g., SOAP).
- Participating as part of a team to formulate an Individualized Care Plan, monitor Veteran progress, and engage in team problem solving.
Ozanam Manor provides transitional housing for homeless elderly and disabled individuals. Residents experience a wide variety of medical and cognitive issues. The focus is to identify and address healthcare issues that affect the ability to obtain and maintain permanent housing. This includes:
