Domiciliary Mental Health Registered Nurse

The Domiciliary For Substance Use Disorders is a 24 bed 24/7 residential treatment program located at the Phoenix VA Main Campus. The domiciliary Mental Health (MH) Registered Nurse (RN) is responsible for providing and leading the provision of competent, evidence-based care to assigned patients in a dynamic patient care setting. The domiciliary RN serves as a charge RN, completes admissions, discharges, care plans, assessments including withdrawal scales, vital sign monitoring, lab collections, sick triage, nursing education, orientation and competencies.

They are responsible for quality improvement and outcomes utilization consultation. The domiciliary RN provides patient-centered and recovery-oriented hands-on direct care and maintains self-awareness while caring for patients living with substance use disorder and/or MH conditions. They facilitate shift report, warm hand-offs, and nurse-led psycho educational groups; use therapeutic communication techniques, delegate to unlicensed assistive personnel, and practice de-escalation techniques using Prevention and Management of Disruptive Behavior (PMDB).

The domiciliary RN utilizes recovery-oriented language, and provides oversight of the patient safe medication management program including medication administration. The domiciliary RN actively participates in MH interdisciplinary treatment team, patient safety and environment of care rounds, and responds to psychiatric and medical care needs and emergencies as directed. The domiciliary RN reports safety concerns, and documents nursing assessments, risk screens, nursing plan of care, patient safety rounds, and environment of care rounds according to facility policy.

The domiciliary RN maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Maintains BLS and PMDB certifications and completes hands on and computer-based clinical trainings as assigned regularly.

Floats to other units based on operational needs of the service. Questions clinical practices for the purpose of providing evidence-based care.
