Position:Licensed Vocational Nurse (LVN)
Reports To: Team Leader
Hours
:
Monday to Sunday 5 pm to 8 am & Monday to Thursday 5 pm to 8 am
Compensation
: $10 on call rate follow per diem visit rate
We’re looking for an per diem LVN who is compassionate, reliable, and passionate about patient care. In this role, you’ll provide direct care under the guidance of a Registered Nurse, follow the plan of care, and work closely with our interdisciplinary team to ensure each patient receives the support they need.
What You’ll DoProvide hands-on care to patients as allowed by the State Nurse Practice Act
Carry out care plans based on assessments and patient needs
Document visits clearly and on time
Help educate patients and families so they feel informed and supported
Communicate patient needs to the wider hospice team (aides, therapists, social workers, dietitians, supervisors, etc.)
Use equipment and resources safely and responsibly
Support pain and symptom management, and monitor how patients respond
Participate in training, development, and quality initiatives
Assist with other tasks as requested by the RN
What We’re Looking ForGraduate of an accredited LVN/LPN program
At least three years of nursing experience (hospice, community health, or med/surg is a plus)
Active LVN/LPN license in the state
Strong communication and organizational skills
Valid driver’s license and a reliable, insured vehicle
- Must be willing to travel far distances
Current CPR certification
A genuine understanding of the hospice philosophy and the needs of the terminally ill
