Experience:

Minimum of 2 years recent RN experience working with individuals in home health, hospice, long-term care, or case management.

License:
Registered Nurse licensed to practice in the Client, active and in good standing without restrictions.

Certifications:

Additional training or certification in behavioral health, complex medical conditions, or nursing case management preferred.

Must-Have:

  • Excellent communication skills, including advanced abilities in explaining complex information.
  • Proficiency in Google Office, Microsoft Office, and Adobe Acrobat.

Description:

We are seeking the support of contract nurses to assist with the rising nursing caseloads experienced across the state. Locations will vary, with the position determined after candidates are selected. The role involves conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered. These assessments are used to authorize skilled nursing services for members. The successful candidate will conduct in-person visits statewide while monitoring health needs, assisting with referrals, and advocating for members' rights.

The Registered Nurse will complete a report for each member, documenting the medical necessity for skilled services tailored to individual needs. In-person assessments are required every 90 days and as needed. The position is field-based, and travel is required to complete in-person assessments. Responsibilities include completing nursing referrals, placing vendor calls, and executing special projects as necessary. Compliance with documentation deadlines and participation in interdisciplinary planning meetings is required to determine and identify appropriate services.

The registered nurse will collaborate with members and their support systems. Attendance at mandatory meetings and completion of required annual training is also expected.
