Licensed Practical Nurse

Aveanna Healthcare is seeking Licensed Practical Nurses to provide compassionate in-home care for pediatric and adult patients. In this role, you will deliver skilled nursing services, including medication administration, wound care, and ongoing monitoring based on individualized care plans. You will document assessments, coordinate with RNs and therapists, and support families in managing complex medical needs. Responsibilities

• Provide direct nursing care to pediatric and adult patients in their homes per care plans
• Administer medications, treatments, and wound care as ordered by physicians
• Monitor and document patients' vital signs, progress, and responses to treatment in clinical records
• Assist with activities of daily living and ensure patient comfort and safety
• Collaborate with RNs, therapists, physicians, and families to support coordinated care
• Educate patients and families on disease management, medications, and home care procedures
• Recognize and report changes in patient condition and respond to emergencies appropriately
• Follow infection control, safety, and regulatory standards in all care activities
• Participate in ongoing training, competency evaluations, and quality improvement initiatives
• Maintain professional communication and uphold Aveanna's values of compassion and integrity
