Heartlite Hospice, a part of LHC Group and United Health Group family, is seeking a Hospice Nurse Practitioner to provide palliative care consultation, symptom management, and supportive care for hospice patients. You will work with the Hospice Medical Director and interdisciplinary team to develop and coordinate care plans.
Key responsibilities include conducting face-to-face visits, collaborating with physicians, documenting care, and educating patients and families.
