Prestige Home Health Care LLC is looking for a compassionate and skilled Registered Nurse (RN) to join our dedicated team in Phoenix, AZ. • Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing procedures in a home setting
• Monitor and document patient progress, vital signs, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on disease management, medications, and home care techniques
• Ensure compliance with state, federal, and agency regulations and standards of care
• Maintain accurate and timely clinical documentation using electronic health records (EHR)

• Active Arizona RN license in good standing
• Current BLS/CPR certification required
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Strong knowledge of skilled nursing procedures, wound care, and IV therapy
• Excellent communication, time management, and organizational skills
• Ability to work independently and manage a patient caseload efficiently
• Valid driver's license and reliable transportation for home visits throughout Phoenix, AZ
