Survivor Clinic & Fertility Preservation – Nurse Coordinator
Unique opportunity to join two growing programs at Phoenix Children’s. The first is multidisciplinary Survivor Clinic, supporting active pediatric oncology and neuro-oncology patients, generally 2+ years off therapy. The program, offered as an annual clinic visit for each patient, screens for late effects of cancer treatment, provides education and long-term follow-up, and transitions patients to adult/community care around age 18 or at least 10 years post-therapy.

The second is the fertility preservation team, meeting with newly diagnosed patients, providing education on planned treatment fertility risk, and coordinating fertility preservation interventions.

• Survivor:
  Review patient medical records to create oncology treatment summaries and long-term follow-up care plans; identify risks for late effects and appropriate screening/testing.
• Survivor:
  Coordinate weekly multidisciplinary Survivor Clinic visits and provide education to patients/families about risks for late effects and recommended screening.
• Survivor:
  Serve as the point person between oncology providers, APPs, endocrinology, neuro-oncology, social work, mental health, rehab, nutrition, research, and other team members.
• Survivor: support IRB research approved Survivor Registry study.
• Fertility Preservation:
  Counsel patients/families about treatment‑related fertility risks and preservation options.
• Fertility Preservation: coordinate fertility preservation interventions such as sperm banking, oocyte cryopreservation, ovarian tissue cryopreservation, and testicular tissue cryopreservation. Collaborate with outside institutions to facilitate care.
• Fertility Preservation:
  Support IRB research approved reproductive tissue freezing studies.

• RN with minimum 3 years of experience; oncology background required.
• Strong knowledge of oncology/chemotherapy protocols and long‑term follow-up guidelines.
• Highly organized with strong communication, coordination, and patient education skills.
• CPHON or CPON certification preferred.

Incumbent will be working in a clinic environment, M – F, 7 AM – 4 PM; NO holidays and NO Weekends!

This position coordinates the patient care services, workflow, and activities provided by nursing and support staff of assigned program. This position provides guidance, support, and direction to staff to ensure consistent provision of developmentally appropriate quality patient care in accordance with all applicable federal, regulatory, and professional standards and requirements.

• Acts as a communication liaison between patient, family, and interdisciplinary team by providing teaching and support to recognize limitations and strengths within the patient/family system.
• Assists with identification of staff and community learning needs from a group and individual perspective. Provides staff and community education on new skills, equipment, policies, protocols, and/or guidelines.
• Identifies and acts upon potential risks, investigates and documents follow‑up on significant incidents, and involves appropriate resource(s) to ensure timely resolution and implementation of corrective measures in accordance with professional, regulatory, and governmental agencies and standards.
• Maintains appropriate records to facilitate accurate data collection and prepares required grant reports.
• Manages the daily operations of a program. Ensures clinic and program effectiveness. Develops, implements, and monitors plans for ensuring the highest quality of care.
• Provides developmentally appropriate clinical care within professional domain to ensure delivery of quality and developmentally appropriate patient care.
• Regularly reviews research‑based literature to improve clinical care for specific clinical populations as well as compliance with regulatory bodies.
