You’ll make an impact by:
  • Partner with nephrologists, dialysis teams, and interdisciplinary care providers to deliver comprehensive dialysis care, address patient concerns, and ensure accurate, timely clinical documentation.
  • Manage chronic kidney disease patients across outpatient settings by conducting assessments, developing treatment plans, and coordinating ongoing care in collaboration with physicians.
  • Facilitate seamless transitions of care through annual wellness visits, post-hospital follow-up, medication reconciliation, and patient education to improve outcomes and reduce readmissions.
  • Provide advanced clinical care across multiple settings—including physician offices, skilled nursing facilities, rehabilitation centers, dialysis units, and on-call environments—through diagnostic evaluations, treatment planning, and evidence-based interventions.
  • Prescribe medications and therapeutic treatments within scope of practice while delivering compassionate, patient-centered care and maintaining the highest standards of professionalism and clinical excellence.
What You’ll Bring
  • Master’s degree in nursing or related field, or equivalent combination of education and experience (4-10 years)
  • Current Arizona RN license with Nurse Practitioner (NP) or Physician Assistant license
  • Proficiency in Microsoft Office and experience with ICD-9 and CPT coding
  • Strong organizational skills, flexible schedule availability and reliable transportation for travel between sites
  • A patient-first mindset and a commitment to providing compassionate care
