Terros Health.org Terros Health is hiring a Psychiatric Nurse Practitioner for a Behavioral Health Urgent Care and Crisis Diversion position in Phoenix, AZ.Full-Time;
Monday-Friday Outpatient
27th Avenue Health Center:
Near 27th Ave/ Indian School Rd Experience in acute psychiatry, psychiatric urgent care, crisis stabilization/triage, emergency psychiatry, or similar is required

Compensation:

Base Salary (Depending on Years of Experience and Education Level):

PMHNP/MSN: $140K-$145

KPMHNP/MSN/Dual Certification (PMHNP/FNP): $148K-$155

KDNP: $158K-$165K Additional Salary:$7/hour = $14,560 Annually Because this specialized position provides critical, high-impact care within our Behavioral Health Urgent Care & Crisis Diversion program, it includes a dedicated behavioral health urgent care specialty premium on top of the base salary.

Language Differential:$3/hour = $6240 Annually Additional Language Differential Pay Available:

- Behavioral Health Urgent Care & Crisis Diversion provides rapid psychiatric assessment, treatment, medication management, and short-term stabilization for individuals experiencing urgent behavioral health needs. Serving as a bridge between outpatient care and crisis services, this role delivers timely interventions, assesses risk and level of care needs, and facilitates connection or reconnection to ongoing behavioral health treatment.

The position also supports transitions of care through hospital and crisis discharge follow-up and collaborates closely with internal clinical teams, Crisis Services, hospitals, and community providers. Services may be provided in person or via telehealth. This role requires the ability to manage a dynamic caseload, respond to unplanned clinical needs, make timely decisions regarding risk and disposition, and provide focused, short-term interventions rather than long-term psychiatric management.

Duties include:

Rapid Psychiatric Assessment and Triage
• Provides timely psychiatric assessment for individuals experiencing acute or emerging behavioral health concerns.
• Evaluates psychiatric symptoms, current treatment, medication needs, substance use, psychosocial factors, medical considerations, and other factors contributing to the current presentation.
• Performs suicide, self-harm, violence, and other relevant safety and risk assessments.
• Determines the appropriate level of care based on clinical presentation and risk.
• Identifies individuals who can safely receive outpatient intervention and those requiring escalation to Crisis Services, emergency department evaluation, 23-hour observation, inpatient hospitalization, detoxification, or another higher level of care.
• Recognizes medical conditions or substance-related presentations requiring medical evaluation rather than outpatient psychiatric intervention.
