Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign checks
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Communicate effectively with residents, families, and healthcare staff to ensure seamless care transitions
- Participate in quality improvement initiatives and maintain accurate and timely documentation
- Collaborate with the healthcare team to identify and address resident needs and concerns
- Maintain a clean and organized work environment that promotes patient safety and comfort
