Signify Health, part of CVS Health, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in their homes and bridging gaps in care.
The visits last about 45 minutes and include a medication/history review, physical evaluation, and optional diagnostics. Travel locally and within licensed states; virtual visits as needed; benefits include health/dental/vision, PTO, CEU allowance, 401K and malpractice coverage.
