Role: Nurse Practitioner / Advanced Practice Provider
Responsibilities- Deliver high-quality primary care to adults on Medicare in underserved communities
- Conduct comprehensive assessments and manage chronic conditions
- Provide preventative care and coordinate with healthcare team members including physicians, nurses, and social workers
- Educate patients and families to support health and wellness
- Utilize data and EMR tools to track and improve health outcomes
- Support and develop care plans tailored to patient needs
- Relevant nursing or advanced practice certification
- Experience in primary care, chronic disease management, and patient education
- Strong communication and teamwork skills
- Ability to use EMR systems effectively
- Commitment to serving underserved populations and improving community health
