Case Manager - Phoenix, Arizona

We are a mission-driven, service-oriented industry leader looking for a Case Manager to join our team of caring professionals in Phoenix, AZ.

Location:

Oak St and 44th St, Phoenix, AZ.

• Assists clients in identifying treatment and recovery goals and provides supportive services to help them reach those goals.
• Completes assessments, crisis plans and individualized service plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with family, behavioral health, medical and dental providers, community resources and others to ensure services prescribed in the plan are implemented.
• Conducts outreach and follow‑up of services including crisis interactions, missed appointments and hospital discharges to ensure resources are in place.
• Performs all case‑management functions associated with caseload, including completing the assessment and service‑planning processes and identifying the need for further or specialty evaluations.
• Pursues best‑practice outcomes for persons with mental illness, including continuing education, employment, independent housing and community tenure.
