Position summary:
Copa Health is seeking a dedicated and compassionate Housing Care Navigator to support individuals experiencing homelessness in achieving long-term housing stability. In this role, you'll provide trauma-informed care, conduct housing assessments, and connect participants with critical community resources. You will play a vital part in empowering participants to overcome barriers, secure housing, and thrive.
Schedule:
Full-time Starting pay: $22-$24 DOE What you'll do:
- Conduct intakes and needs assessments for participants.
- Create individualized housing plans tailored to each member's goals.
- Provide trauma-informed, harm-reduction support and crisis intervention.
- Coordinate with landlords, service providers, and community partners.
- Assist participants in securing identification and housing documentation.
- Advocate for tenant rights and educate members on lease responsibilities.
- Facilitate workshops, peer support groups, and orientation sessions.
- Maintain accurate case notes and documentation in the Homeless Information Management System (HMIS).
- Meet consistently with members to address barriers and support housing goals.
- Transport or accompany participants as needed for housing search and resource access.
Qualifications:
- Bachelor's degree in human services, social work, psychology, or related field OR equivalent combination of education and experience.
- At least 1 year of experience working with individuals experiencing homelessness and/or behavioral health needs.
- Ability to obtain a Level 1 fingerprint clearance card within 8 weeks of hire.
- TB test clearance (upon hire and annually).
- Valid driver's license and acceptable driving record.
- Must pass pre-employment drug screening.
