Through our partnership with the largest pediatric and primary care practice in the community, we are expanding access to behavioral health services for children and families within a trusted medical setting. The Case Manager plays a vital role in supporting this population through intake assessments, service planning, care coordination, and ongoing support throughout each client and family’s treatment.

The Case Manager at Roya Health is responsible for intake assessments, service planning, and coordinating the care of clients receiving services. This role involves working closely with clients, their families, and the clinical team to develop, implement, and monitor individualized treatment plans. The Case Manager will assist clients with accessing the resources and services they need to achieve their treatment goals and address social determinants of health.
Key Responsibilities- Conduct comprehensive intake assessments to identify client needs and strengths.
- Develop individualized service plans in collaboration with the client, family, and clinical team.
- Coordinate and monitor the delivery of services, ensuring that treatment plans are followed.
- Participate in Child and Family Teams (CFTs) and Adult Recovery Teams (ARTs) as necessary.
- Advocate for clients to ensure they receive appropriate services and resources.
- Assist clients with navigating social services and community resources.
- Provide crisis intervention and support as needed.
- Facilitate communication between clients, families, and service providers.
- Document all client interactions promptly and maintain accurate case records in accordance with clinic policies.
- Participate in case reviews, team meetings, and supervision sessions.
- Ensure compliance with all applicable laws, regulations, and ethical standards.
- Bachelor’s degree in social work, Psychology, or a related field (Master’s degree preferred).
- Minimum of 2 years of experience in case management or a related role in behavioral health.
- Strong understanding of behavioral health principles and practices.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Experience with crisis intervention and community resource coordination.
- Proficiency in electronic health record (EHR) systems.
- Behavior coach a plus.
The Case Manager position may require sitting for extended periods, using a computer, and engaging in face-to-face communication. Some travel within the community may be necessary. Reasonable accommodation can be made to enable individuals with disabilities to perform the essential functions of the job.
The work environment is primarily clinical. The Case Manager may also conduct home visits and attend community meetings as needed. The role requires flexibility and the ability to handle a fast-paced, dynamic work environment.
