Community Bridges, Inc. is seeking a Client Care Coordinator to manage housing caseloads at shelter or temporary lodging sites in Arizona. You will coordinate care and support services to help individuals and families end homelessness and connect them with PCMH/PCP services as needed.
You will document interactions, report outcomes, and work with a multi-disciplinary team to ensure services align with program contracts.
