EMPACT-SPC, part of La Frontera Arizona, seeks a SMI Case Management Navigator to provide outreach and engagement and coordinate services per the individualized service plan (ISP). You will document progress toward recovery and communicate with families, behavioral health, medical and dental providers, and community resources.
Working hours are Monday–Friday, 8:00 am–5:00 pm. The role requires at least a high school diploma or GED, with a preference for a bachelor’s degree, and at least one year
