Community Bridges seeks a Lead Navigator in Phoenix to support individuals experiencing homelessness. You are in recovery from AOD and/or GMH for at least one year and have 90+ days of Navigator/Peer Support experience.
You will provide outreach, case management, and wrap-around services, coordinating with RBHA and community providers to improve housing stability. You will mentor Navigator II, participate in a multidisciplinary team, and help coordinate entry, housing referrals, and discharge.
