Job Summary

We are seeking a full‑time Pediatric Discharge Planner to join our Case Management team in Phoenix. The Pediatric Discharge Planner coordinates care across the continuum for children and adolescents, collaborating with families, caregivers, providers, and community resources to support safe, timely transitions of care. This role helps patients achieve optimal health outcomes, access appropriate services, and utilize resources effectively while respecting each patient’s and family’s needs, preferences, and right to self‑determination.

Qualifications
  • Bachelor’s degree in Sociology, Psychology, Social Work, or a related behavioral health field required.
  • Minimum of one year of experience in a pediatric behavioral health, mental health, healthcare, or social services setting preferred.
  • Experience working with children, adolescents, and their families is strongly preferred.
  • Valid Arizona Level One Fingerprint Clearance Card required.
  • Current American Heart Association (AHA) BLS certification for Healthcare Providers required upon hire.
  • CPI certification must be obtained within 30 days of hire.
Job‑Specific Duties
  • Provide clerical and administrative support to Case Managers and Social Workers serving children and adolescents.
  • Collaborate with physicians, therapists, treatment team members, patients, families, and caregivers to develop effective continuing‑care and discharge plans.
  • Assist with telephone calls, faxing, copying, and coordination of pediatric discharge plans.
  • Present age‑appropriate aftercare options to patients and families, ensuring they understand available services and next steps.
  • Collect, release, and manage patient information in accordance with confidentiality requirements to support accurate referrals and care coordination.
  • Maintain timely, accurate, and complete patient records.
  • Initiate communication and coordinate services with referral sources, schools, outpatient mental health providers, community agencies, and other care facilities to ensure continuity of care.
  • Arrange admissions, transfers, and placements with hospitals, treatment centers, and other appropriate pediatric service providers.
  • Schedule follow‑up appointments for outpatient treatment and support services.
  • Attend and participate in treatment team meetings professionally and punctually.
  • Build and maintain positive relationships with pediatric community resources, referral partners, and family‑support services.
  • Conduct research and maintain current provider, community‑resource, and referral databases relevant to children, adolescents, and families.

Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CMC Phoenix roles, including but not limited to shift differential and other special pay practices. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CMC Phoenix in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
