Job Summary

The Housing Solutions Case Manager, H20, is responsible for ongoing needs assessments, personal treatment plans, implementation, support, information, referrals, and advocacy for individuals who are homeless, chronically homeless, and at risk of homelessness. The role provides housing transition/navigation services, housing pre‑tenancy and tenancy sustaining services, and ensures trauma‑informed and client‑focused strategies are practiced.

Essential Duties and Responsibilities
  • Maintain a caseload of 1‑15 individuals and meet residents at least twice a month to develop emergency plans and individual housing support plans based on assessment findings.
  • Meet clients in shelters, out in the community, and in their homes.
  • Utilize harm‑reduction interventions and crisis prevention/intervention techniques to keep clients in housing.
  • Practice trauma‑informed care and motivational interviewing with clients.
  • Assist residents in building strengths and recognizing needs respectfully.
  • Conduct health and safety visits, including habitability inspections.
  • Know community resources and refer clients as needed.
  • Organize and facilitate social and recreational activities that promote community involvement and peer support.
  • Collaborate with community partners, outreach teams, health‑care networks, housing authorities, landlords, social workers, and the leasing team.
  • Provide support during voucher briefings, match meetings, and coordinated entry.
  • Attend trainings to increase knowledge of co‑occurring disorders, harm reduction, and cultural competence.
  • Assist with housing searches, applications, accommodations requests, and ensure safe, ready living environments for move‑in.
  • Identify and intervene early for behaviors that threaten housing stability.
  • Coach clients on maintaining key landlord/property manager relationships and life‑skills for stable housing.
  • Respond to crisis situations by prioritizing and implementing appropriate interventions.
  • Help clients obtain required documents for housing (e.g., , birth certificate, income verification, Social Security.
  • Work as a Mercy House team member, attend internal and external meetings, and plan and develop resident programs.
  • Maintain accurate, up‑to‑date documentation including housing plans, case notes, assessments, and call logs.
  • Assist with data collection systems and meet deadlines.
  • Use computers and relevant software (Microsoft Word, Outlook, Excel, PowerPoint).
  • Respond to calls and emails timely; prioritize workload independently.
Qualifications and Experience
  • Bachelor’s Degree in Social Work or related field preferred.
  • Experience in recovery, mental health, and co‑occurring disorders preferred.
  • Applicants with less than a BA/BS but at least 2 years of direct experience with long‑term homeless, low‑income, and diverse populations are welcome.
  • Knowledge of confidentiality, legal and ethical issues in case management; effective documentation and guidelines.
General Requirements
  • A valid AZ driver’s license, proof of insurance, and a reliable vehicle are required.
  • Comfortable using personal vehicle for home visits, shelter visits, and community meet‑ups.
  • Ability to work effectively with diverse populations; plan, organize, prioritize duties; perform crisis intervention; communicate clearly; maintain a positive, professional, and safe environment; and establish effective working relationships.
Physical Requirements
  • Ability to lift, bend, push, and pull up to 50 pounds.
  • Prolonged periods of sitting at a desk and working on a computer.
