Skilled Nursing LPN or RN Phoenixville

We are seeking Licensed Practical Nurse or Registered Nurse to work one-on-one with our pediatric client in an in-home setting in the Phoenixville, PA area. The available shifts are Monday thru Friday or Saturday and Sunday available
. Additional clients in surrounding areas and full-time or part-time positions are also available.

• Must be a caring and reliable individual who has the ability to work in a diverse atmosphere
• Assess patient's need for safety measures and appropriate interventions.
• Implementation of the Nursing Care Plan as outlined by the Primary Nurse.
• Implement medication changes according to verbal and/or written physician orders.
• Administer prescribed medications and treatments.
• Accurately observe and promptly report changes in the patient's physical or mental status to the Nursing supervisor.
• Accurately record nursing observations, activities, and treatments on the appropriate forms.
• Ability to follow a physician's plan of care under the supervision of our Director of Nursing
• Communicate with family members, in office staff and all supporting health professionals

• Current Pennsylvania Skilled Nursing License
• Current CPR
• Must be able to lift at least 50 pounds
• Must pass State and Federal Criminal Background Checks
• Must have up-to-date Physical and Two-Step PPD or Chest X-ray
• Must be able to pass Skilled Nursing pre-employment test
• Form of Transportation to Client's Residence
